If you would like to make an appointment, please contact our office by phone.
Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without 24 hours notice.
Financial Information
We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. All insurance copays are due at the time of service. We accept cash, checks, Mastercard, Visa, Debit Cards, Discover & American Express. We do our very best to give accurate insurance benefit information, however if your insurance does not send the exact payment expected, you will receive a statement from our office, that is due upon receipt. If you have any questions regarding the insurance payment, please contact your insurance company directly. If you have a question about your bill, we will do our best to assist you.
